According to data from the Veterans Health Administration, a patient centered approach to home medical care helps to improve how chronic illnesses are managed. This is a big step forward when it comes to the maintenance of the health of those with a chronic illness. These diseases are very hard to monitor and this study revealed a lot of information that was previously not well understood. It also revealed some surprising information about just how effective a personalized approach to care can be.
The study lasted for over four years, and revealed that a patient centered approach helped with a number of different diseases. These included diabetes, high blood pressure, and high cholesterol levels. Those who were receiving the patient centered approach to their care at home saw that their tests and measurements stayed within the healthy ranges that doctors had assigned for them. Data from over 800 different VA centers were examined in this study. Some of the centers were more effective than others in their approach. Of the 77 clinics that were best able to implement this strategy by the year 2012, significant improvements were made in five of the seven different chronic illnesses that were measured. In those clinics that scored the lowest, improvements were only found in two different categories.
In total, those clinics that had effectively implemented a home care model saw a 5 percent increase in the overall health of their patients.
So, what exactly is a patient centered approach in this case? In this case, it meant that patients were able to receive personalized medical care from their homes without needing to go into the VA whenever care became necessary. This is far different from in-home care, which is a nonmedical type of care. However, although there are some major differences, the personalized model of care proved to be effective in this instance.
This data is yet another reason why we so heavily stress the importance of in-home care. Is it perfect for everyone? Absolutely not. But because it provides a personalized approach to senior care, it is more effective than a lot of other methods of care. For example, although a nursing home provides a very effective type of care for some individuals, it isn’t right for everyone. And because it is a very hand on type of care, those individuals who will benefit from a great degree of independence in their day to day lives do not thrive under this institutionalized type of care. Even though in-home care is nonmedical in its nature, some people do far better with this than they do any other type of care. It all depends on the individual.
